Following the participation of an assistant instructor on a BSAC seamanship course or event, this form is to be used to provide feedback for ongoing personal development. Should their performance merit the recommendation of award of instructor status, it also provides a proper audit trail and quality assurance process.

This form is to be used for all courses that fall under the remit of BSAC Seamanship Group including Boat Handling Instructor and Diver Coxswain Assessor.

Theory sections of the report should be written in line with PAVE Criteria. Practical sections of the report should be written in line with STEP Criteria.

Where elements of the report are not relevant to a specific course or event then they should not be completed (enter n/a for mandatory fields that do not apply).
