This form is to be used by BSAC Centres to record training and apply for qualification cards

When submitting this form the Centre Boss/Lead Instructor must ensure that:

Each student has an individual copy of the training materials
The Unique Reference Number (URN) supplied with the materials is added to the form below for each student
The form includes the BSAC membership number for each student
There is a passport-style headshot of each student
The Centre and Instructor details are complete

Once this form has been processed then the MyBSAC record for each student will be updated and a qualification card sent to them
