Fitness to dive: medical self-declaration
As a diver and member of BSAC you are required to complete a self-declaration medical fitness form, upon joining, and then annually when renewing.

Add up-to-date medical information
When you have logged in to MyBSAC and purchased an Individual Membership, you're ready to add medical information to your profile.
About the medical options
- Self-Certification Medical - the self-certification all divers are required to complete annually. If you can answer No to all the questions then you do not need to do anything further. If you answer Yes to one or more questions you will need to contact a Medical Referee for further advice.
- Medical Referee - a certificate provided by a UK DMC doctor, or a Recreational Diving Services medical certificate.
- HSE Medical - a medical provided by a diving doctor required for commercial purposes (only complete this if you dive at work)
How to add my medical
- Visit the home page of your profile
- Click on the 'Medical Documentation' tab

- Then click on the 'Add Medical Documentation' button

- Choose which type of medical you wish to complete, confirm the details (or upload a file) and hit submit

Your medical is now in the system. It will have 'Pending' status until approved by BSAC HQ.
Repeat the above to add a Medical Referee document or HSE Medical.
- If you upload a Medical Referee you should include page 3 only which has the UK DMC doctor's sign-off, validity period and any restrictions
- If you upload an HSE Medical you should upload a scan/photo of the front page with the doctor's sign-off
Who are the UKDMC?
UKDMC is the national network of medical referees with accredited diving medicine expertise who set the standards of fitness required to dive. They use a uniform set of medical standards which are continually reviewed as new research is published.

What if I want a medical examination?
Medical examinations for fitness to dive
The current self-declaration process has become an established system with the full support of the UK Diving Medical Committee (UKDMC).
Even if you have answered ‘no’ to all of the answers on the Diver self-declaration medical fitness form but you still feel you would like a medical examination for your own peace of mind there are two options for those that want a medical examination:
- a medical examination from your own GP (in addition to the mandatory self-declaration process)
- or a medical examination from a UKDMC referee - and a UKDMC ‘fitness to dive’ certificate can be issued.
Only a UKDMC referee can endorse a UKDMC ‘fitness to dive’ certificate.
