BSAC nationally awarded qualifications include a card sent as part of certification for OWI, AI, NI, Snorkel Instructor and First Class Diver. Use this form to order qualification cards to replace lost cards or cards for existing qualifications that did not include cards. Once you have completed the qualification form area please add the instructor name and their instructor number at the foot of the form. BSAC HQ will then verify that you have completed the course from your record. If you have proof of your qualification scan in and upload as an image to us.

The qualification card will then be issued in due course.
